2008 Annual Membership Categories and Dues
Membership is held by the individual and cannot be transferred to another individual without consent from the current member.

The period for annual dues is the calendar year, January 1 to December 31. When joining during the calendar year, the membership term applies through December 31 of that year.

Listing in the Membership Directory requires receipt of application and payment of dues by January 31.

Active - $80
An individual who works for a physician or group of physicians engaged in the practice of medicine as a legal entity, or as a part of a legal entity. This member's principal role must be one of the following:

• Performing managerial duties involving multiple areas of medical practices.
• Performing administrative tasks involving a single area in medical practices.
• Providing patient care along with performing significant managerial or administrative tasks.

Active Multi-Member Discount - $40
Organizations with multiple active members pay $80 for first member and $40 for each additional member. 

Associate - $120
An individual who works for an organization whose primary function is to provide management or consultative services through a specified agreement for one or more physicians or physician groups. The physicians or physician groups that receive the service must be engaged in the practice of medicine as a legal entity or as part of a legal entity.

Student - $40
A student member is enrolled as a full time student in an accredited program of health care studies and has documented evidence of this enrollment.
